If you've used ChatGPT, Claude, or any large language model for more than a few minutes, you've probably experienced this: the AI gives you an answer that sounds perfect. Confident. Well-structured. Completely plausible.
And completely wrong.

What is an AI "Hallucination"?
In AI terminology, a hallucination is a plausible but false statement generated by a language model. The AI didn't lie intentionally. It didn't "know" it was wrong. It simply produced an answer that sounded right based on patterns in its training data, even though the information itself was inaccurate or fabricated.
Here's the tricky part: hallucinations don't look wrong. They're written with the same confidence and polish as accurate information. The AI doesn't flag uncertainty. It doesn't hesitate. It just delivers the false information as if it were fact.

For example:

Strong prompt: "Provide a brief overview of television development from the 1950s to today, focusing on major milestones like when color TV was introduced, and then the rise of satellite TV. If you're uncertain about any dates or claims, flag them."

2. Stay Vigilant

This is especially critical for high-stakes situations: legal documents, financial analysis, medical information, or anything that could have real consequences if wrong. Trust the AI to draft. Don't trust it to be your final fact-checker. Don't be like Deloitte in this example here.
3. Take Accountability for Your Input
Here's the uncomfortable truth: your results depend on the quality of your input, context, and clarity. If you ask vague questions, you'll get vague (and potentially inaccurate) answers. If you provide no background, the AI will fill in gaps with assumptions.
Effective AI use starts with you. The better your prompts, the better your outputs. The clearer your expectations, the less room for hallucination.

AI hallucinations are real, and they're not going away overnight. But they're also manageable if you approach AI tools with the right mindset.
Use AI as a first draft generator, not a source of truth. Treat it like a highly capable assistant who sometimes gets details wrong. Review its work. Verify its claims. And recognize that the quality of what you get out depends entirely on what you put in.
